[VA disability](https://benefitkarma.com/articles/military-veterans/understanding-va-disability-benefits-a-simple-guide-for-veterans) ratings normally run from 0% to 100%. But some service-connected disabilities create needs that the ordinary rating system does not fully account for.

That is where **[Special Monthly Compensation](https://benefitkarma.com/glossary/smc) (SMC)** comes in.

SMC can provide a higher tax-free monthly payment to Veterans with certain severe disabilities, combinations of disabilities or care needs. It can apply to situations such as loss of use of a hand or foot, blindness, being permanently bedridden or needing another person’s regular help with everyday activities.

The rules can get complicated quickly because SMC is divided into lettered levels. Here is how the benefit works, what the different levels mean and what Veterans can receive in 2026.

If you’re still getting oriented to the broader VA system, start with BenefitKarma’s [Veterans Benefits Guide](https://benefitkarma.com/articles/military-veterans/guide-to-veterans-military-benefits).

## What Is Special Monthly Compensation?

[Special Monthly Compensation](https://benefitkarma.com/glossary/smc) is part of the **Department of Veterans Affairs (VA)** disability compensation system.

It is generally paid in addition to or instead of ordinary [VA disability compensation](https://benefitkarma.com/learn/understanding-va-disability-benefits-a-simple-guide-for-veterans) when a service-connected disability creates one of the special circumstances recognized by VA.

That is an important distinction.

A 100% disability rating is based primarily on how VA evaluates the severity of service-connected disabilities under its rating schedule. SMC looks at certain additional consequences of those disabilities — such as the loss of use of a limb or the need for regular personal assistance.

In some cases, that means a Veteran receiving SMC can receive substantially more than the ordinary 100% disability rate.

## Who Can Qualify for SMC?

There is no single SMC eligibility test.

Instead, VA assigns different SMC levels based on specific conditions and combinations of conditions.

Examples include the loss or loss of use of a hand or foot, severe vision loss, certain combinations of blindness and hearing loss, amputation of limbs, paralysis, being permanently bedridden or needing regular assistance with daily activities.

The qualifying condition generally needs to be connected to military service.

If you are still trying to establish that connection, BenefitKarma's guide to the [difference between a diagnosis and VA service connection](https://benefitkarma.com/learn/difference-between-diagnosis-and-service-connection-va) explains why having a medical diagnosis by itself is not always enough for [VA disability](https://benefitkarma.com/articles/military-veterans/understanding-va-disability-benefits-a-simple-guide-for-veterans) benefits.

## SMC Levels Are Not Simply a Severity Scale

VA uses letters — including K, L, M, N, O, R and S — to identify different types of [Special Monthly Compensation](https://benefitkarma.com/glossary/smc).

It is tempting to think of them as ordinary disability ratings where each letter simply means a more severe disability. That is not quite how the system works.

Each designation has its own eligibility rules.

For example, SMC-K commonly applies to the loss or loss of use of certain body parts or functions. Levels L through O cover specific combinations of serious disabilities and circumstances. Level R can apply to Veterans with particularly severe disabilities who also need substantial personal care. Level S is associated with housebound status.

There are also half-step levels, such as L½, M½ and N½, and SMC-P can provide intermediate or increased rates in certain combinations of disabilities.

That is why two Veterans with the same overall disability percentage can have very different SMC eligibility.

## What Is SMC-K?

SMC-K is one of the most common [Special Monthly Compensation](https://benefitkarma.com/glossary/smc) designations.

It can apply to certain specific losses or loss of use, including some situations involving a hand, foot, reproductive organ, sight, hearing or other bodily functions.

Unlike many other SMC levels, SMC-K is generally an amount added to a Veteran's regular disability compensation.

For 2026, SMC-K pays $139.87 per month.

VA can pay up to three SMC-K awards in addition to basic disability or qualifying SMC payments in some circumstances.

## What Are SMC-L Through SMC-O?

Levels L through O generally involve more serious disabilities or combinations of disabilities.

VA can assign these levels for circumstances including amputations, loss of use of extremities, serious vision loss, being permanently bedridden and needing regular [aid and attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit).

For example, SMC-L can apply to a Veteran who, because of service-connected disability, needs regular help with basic personal needs such as dressing, eating or bathing.

Higher designations can apply when a Veteran has additional or more severe qualifying losses.

The exact criteria become increasingly specific, so a Veteran should not assume a particular level based only on a diagnosis or disability rating.

VA's [current Special Monthly Compensation page](https://www.va.gov/disability/compensation-rates/special-monthly-compensation-rates/) lists the qualifying circumstances for each designation.

## SMC and Aid and Attendance Are Easy to Confuse

The phrase “ [Aid and Attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit)” appears in more than one part of the VA benefits system.

For Veterans receiving disability compensation, needing regular help with basic activities because of service-connected disabilities can qualify someone for certain SMC levels.

But there is also a separate [Aid and Attendance benefit](https://benefitkarma.com/learn/aid-attendance-benefit) connected to VA Pension.

They are not the same program.

Pension-based [Aid and Attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit) is tied to [VA Pension](https://benefitkarma.com/learn/va-pension-a-financial-safety-net-for-veterans-and-their-families) and includes financial eligibility rules involving income and net worth.

SMC based on aid-and-attendance needs is part of disability compensation and centers on service-connected disabilities.

So if you see “ [Aid and Attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit)” on a VA form or benefits page, the surrounding benefit matters.

## What Is SMC-S, or Housebound SMC?

SMC-S is often called the housebound rate.

It can apply when service-connected disabilities substantially confine a Veteran to the home or immediate premises.

There is also a separate route to SMC-S based on disability ratings: generally, a Veteran can qualify when one service-connected disability is rated as totally disabling and other separate service-connected disabilities independently meet the additional rating requirements.

Because the housebound rules are more technical than the name suggests, being able to leave home occasionally does not automatically answer whether someone qualifies.

For 2026, the SMC-S rate for a Veteran with no dependents is $4,408.53 per month.

## What Are SMC-R.1 and SMC-R.2?

The R levels apply in particularly serious cases involving substantial care needs.

SMC-R.1 can apply when a Veteran already meets certain very high SMC criteria and needs regular [aid and attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit).

SMC-R.2 applies when an eligible Veteran needs an even higher level of care.

This is not simply ordinary help around the house. VA uses specific rules to determine whether the required level of personal or health care is present.

These are among the highest SMC payment levels.

## How Much Does SMC Pay in 2026?

SMC payment amounts depend on the designation and, for many levels, the Veteran's dependents.

For a Veteran with no dependents, selected 2026 monthly rates are:

| SMC designation | 2026 monthly rate |
| --- | --- |
| SMC-K | $139.87\* |
| SMC-L | $4,900.83 |
| SMC-M | $5,408.55 |
| SMC-N | $6,152.64 |
| SMC-O/P | $6,877.12 |
| SMC-R.1 | $9,826.88 |
| SMC-R.2/T | $11,271.67 |
| SMC-S | $4,408.53 |

\*SMC-K generally works as an additional payment rather than replacing the Veteran's basic compensation rate.

These rates became effective Dec. 1, 2025.

Veterans with a spouse, children or dependent parents may receive different amounts. VA also has separate rates for intermediate SMC levels.

BenefitKarma’s [VA disability pay-rates guide](https://benefitkarma.com/learn/va-disability-pay-rates) explains current VA compensation rates in more detail, while VA’s official SMC rate table shows the amount for each specific family situation.

## Dependents Can Affect Your SMC Payment

Unlike ordinary [VA disability compensation](https://benefitkarma.com/glossary/va-disability-compensation) at the lowest ratings, many SMC rates vary depending on a Veteran's family situation.

VA's tables include different amounts for Veterans with a spouse, dependent parents or children.

Additional amounts can also apply for extra children, children over 18 who are enrolled in a qualifying school program and a spouse who qualifies for [Aid and Attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit).

If your family situation changes, make sure VA has accurate dependent information. BenefitKarma has a separate guide to [adding a dependent to VA disability benefits](https://benefitkarma.com/learn/overview-of-adding-a-dependent-to-your-va-disability-benefits).

## How Do You Apply for SMC?

How you pursue SMC depends on the reason you believe you qualify.

Medical evidence is especially important. Records should show not just the diagnosis, but the actual loss of function or need for care that supports the SMC level being considered.

For aid-and-attendance or housebound claims, VA uses **VA Form 21-2680, Examination for Housebound Status or Permanent Need for Regular [Aid and Attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit)**.

A health professional completes important parts of the form describing your condition and what help you need with everyday activities.

VA provides [Form 21-2680 and submission information](https://www.va.gov/forms/21-2680/) online.

Other SMC situations may arise as part of a disability claim involving the underlying service-connected condition.

## What Evidence Can Help?

The strongest evidence depends on the SMC category.

For loss of use or serious sensory impairment, medical records should clearly document the extent of the impairment and remaining function.

For [aid and attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit), evidence may address whether you can dress, bathe, eat, use the bathroom, manage safety risks or perform other personal activities without assistance.

For housebound SMC, medical evidence may document how service-connected conditions limit your ability to leave home.

If you receive treatment through the VA system, your medical records may already document some of these limitations. BenefitKarma’s [VA health benefits guide](https://benefitkarma.com/learn/various-va-health-benefits-va-health-insurance) explains how VA health care works and what services Veterans may receive.

A short statement that someone is “disabled” may not tell VA enough. The evidence should describe what the Veteran can and cannot actually do.

## Can You Receive SMC With a 100% Disability Rating?

Yes.

SMC exists specifically because the ordinary [VA disability](https://benefitkarma.com/articles/military-veterans/understanding-va-disability-benefits-a-simple-guide-for-veterans)-rating system does not capture every circumstance.

A Veteran can have a 100% disability rating and still qualify for SMC if the service-connected disabilities meet the additional requirements.

Likewise, some SMC benefits — particularly SMC-K — can apply even when the Veteran's overall disability rating is far below 100%.

So you should not use your total disability percentage alone to decide whether SMC is worth investigating.

## Is Special Monthly Compensation Taxable?

No.

Like ordinary [VA disability compensation](https://benefitkarma.com/glossary/va-disability-compensation), SMC is generally excluded from federal taxable income.

That means Veterans generally do not report these payments as taxable income on their federal tax returns.

## When Do SMC Rates Change?

[VA disability](https://benefitkarma.com/articles/military-veterans/understanding-va-disability-benefits-a-simple-guide-for-veterans) and SMC rates generally receive the same annual cost-of-living adjustment used for Social Security benefits.

The current 2026 rates took effect December 1, 2025.

New rates normally take effect each December after the next cost-of-living adjustment is announced.

Because of that, always check the current VA rate table if you are reading an older SMC article or award estimate.

## The Bottom Line

[Special Monthly Compensation](https://benefitkarma.com/glossary/smc) is one of the more complicated parts of the [VA disability](https://benefitkarma.com/articles/military-veterans/understanding-va-disability-benefits-a-simple-guide-for-veterans) system, but it can also be one of the most valuable for Veterans with serious service-connected disabilities.

The biggest thing to remember is that SMC is not simply another disability percentage.

Eligibility depends on specific losses, loss of use, combinations of disabilities or care needs. A Veteran may qualify for SMC-K with a relatively low overall disability rating, while other SMC levels require much more severe circumstances.

And if you need regular help with daily activities, make sure you know which [Aid and Attendance](https://benefitkarma.com/articles/military-veterans/aid-attendance-benefit) system you are dealing with: VA Pension and disability-based SMC use different eligibility rules.

If your service-connected conditions involve severe loss of function, housebound status or regular personal-care needs, it may be worth reviewing SMC even if you already receive [VA disability compensation](https://benefitkarma.com/glossary/va-disability-compensation).
